End of Life Care at The Fountains at Hosanna is thoughtfully designed to bring comfort, dignity, and peace to individuals in their final days, weeks, or months. Our dedicated team, including a full-time Chaplain, partners with the hospice provider of your choice to ease pain and reduce stress—supporting both the individual and their loved ones through this meaningful time. 

Care is provided in the comfort of each resident’s own apartment, grounded in our core values of dignity, compassion, service, integrity, and innovation. Our on-site Chapel and spiritual care team offer space for reflection and support, with a direct indoor connection to Hosanna Church. 

To honor those who pass, we offer a Words of Comfort service, and a dignity walk—small but powerful moments that recognize each person’s life with respect and love.
