End of Life Care
End of Life Care at The Fountains at Hosanna is designed to fulfill the needs of people in their final days, weeks or months of life. Our full time Chaplain and community staff members can work in conjunction with the hospice provider of your choice. Our goal is to eliminate pain and stress while improving the quality of life for the individual and members of their family and friends. Compassionate care is provided in the comfort of our residents’ own apartment homes. Services emanate from our core values of Dignity, Integrity, Service, Compassion and Innovation. We have our own Chapel and full time Chaplain to support and comfort residents and families. We are also connected to Hosanna Church through an indoor walkway. After a person leaves us, we offer a Words of Comfort Service; our staff shows their affection and respect with a dignity walk.

The Fountains at Hosanna is managed by Ebenezer, Minnesota’s largest senior living operator. Ebenezer is the senior housing division of Fairview Health Services and has 100 years of experience serving older adults.
